Medical Marijuana Certification Process

The following steps outline the process we will go through to determine if you are eligible to obtain a Medical Marijuana Certification.

Step One: Medical Records & Application

• Contact us to obtain the Application and have medical records for your qualifying condition faxed to our office at  (520) 297-9633 or bring them into the office

                        OR

• Complete our Authorization for Release of Records so we may obtain them for you
• If you have no medical records from the past 12 months please contact the office to discuss your options.

Step Two:  Physician Review

• Upon receipt of your medical records, the Staff Physician will review your records to determine if you qualify for the Arizona’s Medical Marijuana Program.
• If additional documents are required for the review, you will be contacted by a staff member.
• If you qualify for the program, you will be contacted to schedule an appointment.

Step Three: Your Appointment

• During the appointment, the Staff Physician will complete a full assessment of your medical history, review your medical records, conduct any pertinent exam(s) and develop a treatment plan that will provide for management and routine care of the debilitating medical condition.
• After review and approval, as a qualified patient you will receive a signed Physician Certification Form that will allow you to apply to the state for a Medical Marijuana Identification Card OR you can choose to have our office staff complete the application process for you by paying a small fee.

Step Four:  Application for Card

• Requires: AZ State Issued ID Card or Drivers License WITH CURRENT ADDRESS, Signed Physician’s Certification Form, signed patient Attestation form, Current photo.
