Individual Licensing Portal

On June 3, 2020, the AzDHS created a new Individual Licensing Portal. This portal provides the patient access to:

  • Medical Marijuana Card
  • List of Dispensaries
  • Purchase History
  • Card History
  • Application Processing and Information

If you would like us to process your medical marijuana application, we will need access to your personal login.  If you have not logged into the portal, we will be asking you to do so before your appointment.  Please follow the instructions below.  If you have logged into the portal, please make sure we have your login information to be able to process your application.

If You are Renewing but have not Received Emails from AzDHS
If you are renewing your card and you have not been receiving emails from AzDHS, we need to make sure the AzDHS has your current email address before you create a log in on AzDHS portal. Please email M2programsupport@azdhs.gov with your name, mailing address, phone number, date of birth and tell them you need to change your email address. Otherwise, if you do not change your email address with AzDHS and create a new account, your new card will be effective immediately and you will lose your current expiration date.

For new applicants:
1. Go to https://azdhs.gov/licensing/medical-marijuana/ or google “individual licensing portal”
2. Click on Individual Licensing Portal on the left side menu.
3. Click on Don’t have an account? Sign up here.
4. Complete the requested information and follow instructions to set password.
5. Bring password to your appointment. We will need it to process your application.

For recertification applicants:

  1. If you would like us to process your application, we will need your Individual Licensing Portal password.
  2. If you have not logged into the Individual Licensing Portal or you forgot your password:
  3. Go to https://azdhs.gov/licensing/medical-marijuana/
  4. Click on Individual Licensing Portal on the left side menu.
  5. Click on Forgot Password
  6. Enter your email address as your username.
  7. Follow instructions and create a new password.
  8. Bring password to your appointment.  We will need it to process your application.
NEED HELP? If you do not have a computer, a smartphone, or need help with the new portal and electronic card, we will help you create an email and/or portal login and print a copy of your card or download your card to your smartphone for a fee of $15.00. In most cases, it would require the patient to return to the office for a second visit.

Access Electronic Card and Portal
To access the portal again, go to https://www.azdhs.gov/licensing/medical-marijuana/index.php
Click on the Individual Licensing Portal and enter your username (email address) and password.
Click on the box Medical Marijuana Patient.
Click on My ID Cards
Click on Download PDF
Print out Card or Save on your smartphone or computer for future use.

Application Discrepancies
If your application has a discrepancy, immediately call us at (520) 297-9664 and forward the email to stephanie@drsadilek.com. We will find out what the issue is and take care of it. Usually this is only a typo on the application, but it needs to be fixed within 15 business days.

Waiting for Medical Records
If we have to request your medical records from your doctor(s), this may delay your application being processed. Medical records can take up to 30 days to receive from your physician. Once we receive your medical records showing your qualifying medical condition, we will start the application process.

AzDHS Receipt and Approval Emails
You will be receiving emails from AzDHS.
1st email will state that your application was received and attached will be your receipt for your Visa or MasterCard. If you paid cash or with Discover or American Express, you will not be receiving this email.
2nd email will state that your application was approved, and you will receive another email in a few days with a link to get your digital card. If you don’t receive the link, please call the state at 602-364-1793 or email them at M2programsupport@azdhs.gov. You can always go onto the individual licensing portal to check the status of your application.

Invalid Renewal
Your application has been approved, but you have accessed your online card and the expiration date has not changed. It will change to your new expiration after the current card has expired. Your renewal card will be listed as invalid renewal until the expiration of your current card.
